I. Purpose & Scope

To provide a detailed work instruction on the requirements for filling out the Civil Prefield Form.

III. Process

  1. Enter the PM Number and the actual address the work will be completed in the applicable fields. Enter the GPS coordinates if physical address is not applicable.
  2. Enter the date work is scheduled to be completed.
  3. Enter the date clearance is to be performed.
  4. Enter a brief description of the overall job scope.
  5. Enter who pre fielded the job as applicable.
  6. Enter the current USA# and notate the expiration date.
  7. Enter the most recent team lead for the job. If job is changing hands enter the last working team lead’s name.
  8. Mark appropriate selections for the following;
    • Pictures (ensure pictures are in a location available to all employees to access).
    • No parking signs (verify the date of no parks)
    • T-1 Excavation
    • QEW Required (List name of the QEW in the notes section)
    • Drain inlet protection needed
  9. If there is traffic control list the number of flaggers to be on site.
  10. If there is a T-1 excavation list the Width, Length and Depth of the excavation in the appropriate fields.
  11. List the address of the homeowner in the appropriate field.
    • Select if the homeowner has been notified, if not list in the notes if they will need to be notified or if there is a phone number available for the homeowner.
  12. In the notes section, enter any appropriate information necessary for safe access and work conditions. (i.e. Dirt road muddy/floods when rains, narrow roads, access gate codes, animals etc.)
  13. Enter the amount of time it takes to travel from the yard to the job site location.
  14. List any additional safety concerns in the appropriate field such as work that might impact customer structures or systems or any hazards such as securing electric gates, fences, streetlight poles, traffic loops etc.
  15. For jobs that have multiple locations enter each location and the scope of work to be performed in the applicable fields.
  16. Select if the the job has a primary enclosure change out or not.
  17. List the name of the employee who completed the pre-dig.
  18. If yes was selected for primary enclosure change out, select the applicable closure size and type.
  19. Select if the job is has a secondary enclosure change out or not.
  20. If yes, select the applicable closure size and type.
  21. If placing a primary TX pad list the relevant information in the applicable field.
  22. If existing pad is in location list the pad size.
  23. If placing a new pad list the new pad size.
  24. Enter any relevant information relating to removing/using or replacing the existing pad or placing a new pad.
  25. Enter the lid/lid frame type and size.
  26. Enter any relevant information relating to removing/placing or replacing the lid/lid and frame.
  27. If soil type is known, select the applicable soil type.
    NOTE: for multiple locations list this information as known, with the scope.
  28. Select the applicable landscape type.
    NOTE: for multiple locations list this information as known, with the scope.
  29. If additional information such as customer specifications, sprinkler lines, etc. is known, list in the Additional Restoration Notes section.
  30. List additional pertinent information required to safely perform the job in the project notes section.
  31. List the scope of work last performed on the job and what is to be completed in the Scope of work section.
